Here you will find the required Patient Registration paperwork that can be filled out when you attend your first session at Inner Resources Counseling. For your convenience, we have also provided them here to be downloaded if you prefer to download and complete them ahead of time. 

If you are engaging in telehealth services, this paperwork must be completed and returned to your therapist prior to the day of your first session.

The Release of Information Form needs to be completed only if you would like us to communicate with a third party about our counseling work. Otherwise, any therapy attended is confidential.

The No Surprises Act & Fee Information needs to be completed only if you are paying privately and not going through insurance.
